I am
writing to inform you of an error that I have spotted in my insurance policy.
The plan that I received last week states that I have only one beneficiary. I
don't know what could have happened, but I did, in fact, submit two names and
two separate sets of documents to cover two beneficiaries.
The primary beneficiary, whose name already appears in the policy, is my daughter while the second beneficiary is my youngest son. Please go over the attached copy of his records.
